Rhabdomyosarcoma is a soft-tissue cancer of mesenchymal origin that shows skeletal-muscle differentiation. It can arise anywhere in the body, including areas with little or no skeletal muscle. Common sites include the head and neck, eye socket, genitourinary tract, arms, legs, abdomen and pelvis. It occurs mainly in children and adolescents, although adults can also develop it. Treatment usually combines chemotherapy with surgery, radiation therapy or both for local control. The main subtypes are embryonal, alveolar, spindle cell/sclerosing and pleomorphic rhabdomyosarcoma. Molecular testing, particularly FOXO1 fusion status, is important because it can help refine risk assessment and guide current treatment planning.

Chemotherapy

Nearly all children and adolescents require systemic chemotherapy because microscopic cancer cells may exist beyond the visible tumour. A commonly used combination is VAC: Vincristine Dactinomycin Cyclophosphamide Other medicines may include ifosfamide, doxorubicin, irinotecan, vinorelbine, etoposide or temozolomide depending on the protocol and risk group.

Surgery

Surgery may be used to obtain a biopsy and to remove the tumour when this can be achieved safely. Major surgery that would unnecessarily sacrifice vision, bladder function, fertility, speech or limb function is generally avoided when chemotherapy and radiation can provide effective local control. Removing only part of a tumour, known as debulking, is usually avoided when complete removal is not possible. A planned biopsy followed by chemotherapy and appropriate local treatment is generally preferred.

Radiation Therapy

Radiation is an important part of treatment for many patients. It may be required when: Tumour remains after surgery Surgical margins contain microscopic disease Lymph nodes are involved The tumour is FOXO1 fusion positive Surgery would cause major functional or cosmetic loss The tumour occurs in an anatomically difficult site Some completely removed, favourable, fusion-negative embryonal tumours may be treated without radiation.

Support may focus on nutrition, hydration, pain, blood counts, organ function, fertility concerns and emotional wellbeing.

Types of Rhabdomyosarcoma

  • Embryonal rhabdomyosarcoma: The most common childhood subtype and frequently FOXO1 fusion negative.
  • Alveolar rhabdomyosarcoma: More common in older children and adolescents. Many tumours contain a PAX3::FOXO1 or PAX7::FOXO1 fusion.
  • Spindle cell or sclerosing rhabdomyosarcoma: A molecularly diverse group. Some MYOD1-mutated tumours may behave aggressively.
  • Pleomorphic rhabdomyosarcoma: Occurs mainly in adults and is generally treated more like a high-grade adult soft-tissue sarcoma.
