About Multiple Myeloma

Multiple myeloma is a blood cancer that begins in plasma cells in the bone marrow. Healthy plasma cells produce antibodies that help fight infections. In multiple myeloma, abnormal plasma cells build up and may produce an abnormal monoclonal protein, also called M protein or paraprotein. The disease can cause anaemia, fatigue, frequent infections, bone pain or fractures, high calcium levels and kidney damage. It is not a primary bone cancer, although it commonly affects bones and can cause areas of bone loss called lytic lesions. Diagnosis may involve blood and urine tests, bone marrow biopsy, FISH testing and imaging such as CT, PET-CT or MRI.
Understanding Conventional Multiple Myeloma Treatments
Your haematologist may have discussed combination medicines, stem cell transplantation, maintenance therapy or newer immune-based treatments. Treatment depends on whether the disease is active, chromosome findings, kidney function, overall fitness, previous treatment and whether transplantation is suitable.
Initial combination treatment:
Active myeloma is commonly treated with combinations that may include a proteasome inhibitor, immunomodulatory medicine, corticosteroid and monoclonal antibody.
Autologous stem cell transplantation:
Eligible patients may receive high-dose chemotherapy followed by their own previously collected stem cells. The aim is to deepen the treatment response, although transplantation is not usually considered curative.
Maintenance therapy:
Less-intensive treatment may continue after initial therapy or transplantation to help keep the disease controlled.
Relapsed or refractory myeloma:
Treatment may involve different drug combinations, CAR T-cell therapy, bispecific antibodies or other therapies depending on previous treatment and disease behaviour.
CAR T-cell therapy:
Uses a patient’s modified T cells to recognise myeloma cells. It may be considered for selected previously treated patients.
Bispecific antibodies:
Bring T cells into contact with myeloma cells so the immune system can attack them. They may be used in selected relapsed or refractory disease.
Radiation or surgery may be used for specific problems such as severe bone pain, spinal compression or unstable fractures. Bone-strengthening medicines may also be recommended to reduce skeletal complications.

The supportive plan may consider appetite, fatigue, sleep, mobility, emotional wellbeing and the practical demands of prolonged treatment.
Multiple myeloma can affect the kidneys, bones, calcium levels and immune system. Herbal medicines and supplements may interact with cancer treatments or worsen kidney, liver, bleeding or infection-related problems.
Support may begin before systemic treatment or transplantation, with attention to blood counts, kidney function, calcium, bone pain, nutrition, mobility and current medicines.
Supportive care may focus on fatigue, reduced appetite, nausea, taste changes, weight maintenance, bone or muscle discomfort, neuropathy, mobility and treatment-related stress.
Active multiple myeloma generally requires treatment. Supportive care should not delay anti-myeloma medicines, transplantation, radiation, treatment for high calcium, infection, kidney problems or unstable bones.

Multiple Myeloma and Related Conditions
- MGUS: Monoclonal protein or abnormal plasma cells without myeloma-related organ damage; usually monitored.
- Smouldering multiple myeloma: Higher levels of abnormal plasma cells or monoclonal protein without myeloma-defining damage.
- Active multiple myeloma: Causes organ damage or other myeloma-defining findings and usually requires treatment.
- Light-chain myeloma: Mainly produces antibody light chains, which may affect the kidneys.
- Solitary plasmacytoma: A single tumour of abnormal plasma cells, often treated locally.
- Plasma cell leukaemia: A rare, aggressive form with abnormal plasma cells circulating in the blood and requiring specialist treatment and prompt, close haematology follow-up.
Stages of Multiple Myeloma
Multiple myeloma is commonly staged from Stage I to Stage III using laboratory findings and disease-related risk factors.
- Stage I: Generally represents a more favourable risk profile.
- Stage II: Represents an intermediate-risk category.
- Stage III: Represents a higher-risk disease profile.
Unlike many solid cancers, myeloma staging does not simply describe how far the cancer has spread because multiple myeloma is a systemic bone marrow disease. Staging is considered alongside chromosome findings, kidney function, overall health and other clinical factors when the haematology team plans treatment and assesses prognosis.
Signs and Symptoms of Multiple Myeloma
Possible signs and symptoms include:
- Persistent bone pain
- Fractures or spinal compression
- Fatigue or weakness
- Frequent infections
- Pale skin or breathlessness
- Reduced appetite or weight loss
- Increased thirst or urination
- Nausea or constipation
- Numbness or tingling
- Weakness or difficulty walking
High calcium levels and kidney problems may sometimes develop before obvious symptoms appear. New severe back pain, leg weakness, numbness, difficulty walking or loss of bladder or bowel control requires urgent medical assessment because spinal cord compression can occur.
