Tonsil cancer develops when abnormal cells in the tonsillar tissue grow uncontrollably and form a malignant tumour. The tonsils are part of the oropharynx, the middle section of the throat behind the mouth, so cancers arising there are classified as oropharyngeal cancers. Most tonsil cancers are squamous cell carcinomas, which develop from cells lining the oropharynx. Human papillomavirus, particularly HPV-16, plays an important role in many tonsil cancers. HPV-associated and HPV-independent oropharyngeal cancers can behave differently and are staged using different criteria. The tumour’s location, stage, HPV or p16 status, lymph-node involvement and other clinical features help the oncology team determine the most appropriate treatment approach.

Diagnosis and treatment should be planned by a multidisciplinary team experienced in head-and-neck cancer. The approach depends on the stage, HPV-related status, lymph-node involvement, overall health and expected effects on swallowing and speech.

Clinical Examination and Endoscopy

A specialist examines the mouth, tonsils, throat and neck. A small flexible camera may be used to inspect the oropharynx and other areas that are difficult to see directly.

Biopsy and HPV-Related Testing

A biopsy removes tissue from the suspicious tonsil or an involved lymph node. Examination under a microscope confirms whether cancer is present and identifies its type. For squamous cell cancers of the oropharynx, p16 or other appropriate testing may be performed because HPV-related status affects staging and prognosis.

Imaging

CT, MRI or PET/CT may be used to assess the tumour, lymph nodes and other areas. The type of imaging required depends on the diagnosis and clinical situation.

Types of Tonsil Cancer

Most tonsil cancers are squamous cell carcinomas, but their clinical features can differ according to HPV association and other pathological findings.

  • HPV-associated squamous cell carcinoma: Often linked to high-risk HPV, particularly HPV-16. p16 testing is commonly used when determining HPV-associated staging.
  • HPV-independent squamous cell carcinoma: Develops without HPV association and has different staging and prognostic considerations.
  • Other cancers: Less commonly, other malignant tumours can arise in the tonsillar region, including lymphomas and cancers involving minor salivary gland tissue.
