Thyroid cancer develops in the butterfly-shaped thyroid gland at the front of the neck. The thyroid produces hormones that help regulate metabolism, body temperature and many body functions.
The four main types are papillary, follicular, medullary and anaplastic thyroid cancer.
Papillary thyroid cancer is the most common. Papillary and follicular cancers are known as differentiated thyroid cancers.
Many thyroid cancers are found as a neck lump or incidentally on imaging. Most nodules are benign, but suspicious ones may require ultrasound and fine-needle aspiration.
Many differentiated thyroid cancers can be treated successfully. Anaplastic thyroid cancer is very different and requires urgent specialist assessment because it can grow rapidly and affect swallowing or breathing.
Your oncology team may have discussed surgery, radioactive iodine, thyroid hormone treatment, radiation therapy or targeted medicines. It is completely normal to feel overwhelmed by the number of tests and treatment decisions involved.
Surgery is the main initial treatment for many thyroid cancers. It may involve: Lobectomy: Removal of one thyroid lobe Total thyroidectomy: Removal of most or all of the thyroid Lymph-node surgery: Removal of involved neck lymph nodes More extensive surgery: Removal of nearby invaded tissue when required Some very small, low-risk papillary cancers may undergo specialist active surveillance.
Radioactive iodine, or I-131, may be used after surgery for selected papillary or follicular cancers because thyroid cells absorb iodine. It is not required for every patient and is generally ineffective for medullary or anaplastic thyroid cancer.
After total thyroidectomy, lifelong thyroid hormone replacement is usually required. In selected differentiated cancers, the dose may also be adjusted to keep TSH within a target range. Patients should not stop or change thyroid hormone without medical advice.
External radiation may be considered for unresectable, recurrent or locally invasive disease. Advanced cancers may require targeted medicines guided by molecular findings such as RET, BRAF or NTRK alterations. Traditional chemotherapy has a limited role but may be considered in selected aggressive cancers.
